Make a professional referral

Make an adult social care referral or report a safeguarding concern if you’re a professional such as a doctor, nurse, social worker, police officer or housing officer.

Information we need to accept a referral

We need enough information about someone to be able to identify them and contact them. 

We’ll ask for:

• personal details like their name and date of birth
• medical information like their GP and NHS number
• details of their next of kin
• what they need help with

We will only be able to help someone you refer to us if we have enough information about them, and if we are the right organisation to help them.

Try to speak to the person before making a referral to us. We may not be able to help someone who does not want it.

Report abuse or neglect (safeguarding concern)

Use this form to refer an adult with care and support needs who is at risk of abuse or neglect.

If the person is under 18, refer them to child safeguarding

Care Act, occupational therapy or DST referral

Use this form to refer an adult for:

• Care Act assessment (care needs assessment)
• occupational therapy assessment
• joint assessment with their carer when they both have care needs
• Decision Support Tool (DST) - for NHS continuing healthcare administrators only

Before you make a referral, make sure the person you’re referring has eligible care and support needs.

Mental health referral

We cannot urgently help anyone who is in crisis or immediate danger of hurting themselves or someone else. 

If someone is having a mental health crisis or having suicidal thoughts:

• contact Kingston’s NHS mental health crisis line by calling 0800 028 8000
• refer them to the Kingston Single Point of Access (KSPA) on 0203 513 1733 or KingstonSPADuty@swlstg.nhs.uk

Refer to the mental health social care team

Refer someone to the mental health social care team if they either:

• have eligible care and support needs which are caused by their mental illness
• need Section 117 aftercare

If the person has dementia but no other mental health need, use the general Care Act referral form.

Drug and alcohol problems

Refer someone with drug and alcohol abuse problems to Kingston Wellbeing Centre

Council housing

You can refer people directly to council housing services without contacting adult social care, such as:

• council housing and supported living for people up to 54
• housing support for vulnerable people aged 55 or over
