How our specialist Community Learning Disability Team (CLDT) can help people with learning disabilities and their friends and families.
The Community Learning Disability Team are a social care team that supports adults with learning disabilities and their families or carers.

We have lots of resources to support people with a learning disability in having independent, safe, and active lives.
We might be able to help with things like:
If you contact us, we will ask you questions about you and how you live. We want to find out what you need help with, and if we are the right team to help you.
We can give you advice about other organisations that can help you or things that you might want to try.
If we think you need more help and support, we will recommend that you have a care needs assessment with a member of our team. This usually means that we’ll come to your home to talk to you and ask you questions about how you live.
The care needs assessment is a longer conversation to work out if you are eligible for funded care from us.
The care needs assessment helps us work out if you are eligible for ongoing funded care from social care.

Speaking to us and getting an assessment is free. If you are eligible for ongoing support, most people have to pay towards the cost of their care. Find out more about paying for social care.
